INSTRUCTIONS FOR NUCLEAR STRESS TEST

Please come in to the office on _____________________2005
at _______________ and the nuclear stress test will begin at _________________.

Instructions:

1. Do not eat after midnight the day of the test or consume any beverages containing caffeine 24 hours before the test.
2. On the day of the test take you medication in the morning with a cup of water.
3. Bring a snack on the day of the test.
4. If you need to cancel this test you must notify our office no later than the Thursday before your test, otherwise there will be a charge of $150.00 for the medication.
